Parental Consent for Photos*

The Jazz Centre/VSC uses visual and audio-visual recordings for promotional purposes. Photos and videos of classes, rehearsals, performances and any other studio events may be made public on printed marketing material, our website and social media. Please indicate if you give consent for your child to participate.

Terms and Conditions*

I have read and agree to the Terms and Conditions and agree to abide by them.

FEES
-Upon enrolment each family will be sent an invoice for the the full year of dance fees.
-A payment plan will be set up on a monthly or by term basis depending on preference.
-This will be the weekly price multiplied by 40 weeks, along with registration, costume and production levies. This will then be divided over a 10 month or 4 term period.
-Payment plans are arranged using Integrapay. Additional fees may apply.
-Parents will be given a link through Integrapay where you will be required to give bank details.
-Students joining mid-term are charged on a pro-rata basis.
-Payment plans must be requested in writing, prior to the due date.

REFUNDS & MISSED CLASSES
-Refunds will not be provided for missed classes or withdrawal from classes.
-If a student misses a class, they may attend an appropriate make-up class.
-Where The Jazz Centre cancels a class, a make-up class or credit will be offered.
-Students may request a credit for missed classes due to any long-term injury or illness.

COMMUNICATION
-Email is the primary form of communication at The Jazz Centre.
-It is the responsibility of the Parent/Guardian to check their emails regularly.
-Should you not be receiving our emails, junk/spam email should be checked.
-It is recommended you add thejazzcentre@gmail.com to your email contacts.
-Facebook and Instagram are not accepted as valid methods of communication for any administrative purposes. These should be communicated via email.
-Parents/guardians must update The Jazz Centre in writing with any changes to personal information that was previously provided on their enrolment form.
-Parents and teachers are not to pass out private contact information or seek access to each other’s profiles on social media platforms.

HEALTH AND SAFETY
-Students participate in classes at their own risk.
-The Jazz Centre accepts no liability for any injury that may be incurred while participating in The Jazz Centre activities or visiting the The Jazz Centre premises.
-The Jazz Centre reserves the right to arrange medical transportation, at the student’s expense, in the event of an emergency.
-While not participating in dance class, children remain the responsibility of parents/guardians at all times. This includes siblings.
-Students with medical conditions must bring any required medical equipment to every class e.g. Epipens, Insulin, puffers etc.
-Students are to be collected from the studio. Students are not permitted to wait in the carpark or on the street.
-Students must not walk outside in dance shoes or bare feet.
